Dr. Blake graduated from Medical University of South Carolina and the School of Aerospace Medicine (USAF) where he was a Strategic Air Command Flight Surgeon.  He was awarded the SAC Flight Surgeon of the Year Medal and the Vietnam Service Medal among others.  Dr. Blake was Chief of Orthopaedic Surgery in 1993.  He is listed as a Fellow with the American Academy of Orthopeadic Surgeons and the American Academy of Disability Evaluating Physicians.  He is also board certified with the American Board of Orthopeadic Surgeryand the American Board of Independant Medical Examiners.
