PAYMENT POLICY
We participate with most major insurance companies. We will file your insurance claim for you.  However, co-pays, co-insurance and deductibles are expected at the time of service.

NEW PATIENTS
Upon your arrival, you will be asked to fill out our required demographic and medical history forms. You may also print these forms and fill them out in advance to bring with you to your appointment.  Also, please bring your medications, insurance cards, and any relevant
x-rays.

CANCELLATIONS
If you must cancel your appointment, please notify us as soon as possible. Your courtesy will allow us to serve another patient who may otherwise have to wait.
